1. You must have received an email from ezyVet Marketing inviting you to partake in this survey to be eligible for the gift card.
2. You must be the owner, practice manager or directly responsible for choosing practice management software at your hospital.
3. You must submit your feedback by 15th October 2022.
4. Please allow 30 days upon your submission of feedback for your gift card to be emailed to you.
